Please remember to bring your Insurance and Photo ID on the day of your appointment. Insurance regulations requires that your Insurance and photo ID are presented on each day of service. Also, please consider that the majority of Insurance companies require pre-certification or prior-authorization for certain procedures and it’s in your best interest that you have your Insurance benefits and eligibility checked prior to your office visit. In addition, please check if a referral is required for specialty care; if a referral is required, one must be brought with you on your initial date of service. To ensure that all Insurance information is complete for each patient, we encourage you to provide certain personal demographics when scheduling your appointment with us; this will include Insurance ID, Birthdate and Home Address.
